Therapy with Annelise

Prospective or Existing clients can log in to the portal to schedule an intake call, view their upcoming appointments, cancel appointments or request a different time. You can also view invoices and update your contact information. To update credit cards people use your Ivy Pay link from your most recent receipt or ask me to send a link. If it's been 3 months or more since our last session, please contact me directly to re-activate your profile.

Feedback and Privacy

Your feedback matters and is always welcome. If you'd like to share your experience, you are welcome to reach out to me directly. That is always my preference and keeps our communication private.

Some clients choose to share their experience publicly online. If you choose to do so, please keep in mind that public reviews may reveal that you receive or have received services here. Please do not include any personal health information. I am unable to respond to public posts in any way that acknowledges or discusses your care due to confidentiality obligations.

Leaving any form of feedback is completely optional and has no bearing on your care.

GOOD FAITH ESTIMATE AND FEE SCHEDULE

The Number of Sessions Will Be Determined as We Progress

 

Under the No Surprises Act, which went into effect in January 2022, all healthcare providers are required to provide clients who do not use their insurance with a Good Faith Estimate of expected costs. This estimate gives us an opportunity to have an open conversation about fees and find an arrangement that works for you. I have provided a fee schedule which will allow my clients and me to determine an approximate total of fees they will pay over the course of 1 year of treatment.

 

CPT CODE 90834 Psychotherapy, 50 minutes $210

CPT CODE 90853 Group Psychotherapy $100

Cancellation Fee: Your therapist requires 24-hour notice for cancellations. You are responsible for the fee of any appointment missed with less than 24-hour notice.

 

Total Estimate: This Good Faith Estimate explains your therapist's rate for each service provided. Your therapist will collaborate with you throughout your treatment to determine how many sessions you may need to receive the greatest therapeutic benefit based on your diagnosis or presenting clinical concerns. You are only responsible for payment for sessions that have taken place, have been late cancelled, or are considered a no-show. A no-show is defined as a scheduled session in which the client did not arrive and did not provide notice prior to your therapist reaching out at the time of the appointment.
